Preschool Application and Required Forms

Registration forms for Asbury Community Christian Preschool are available to print below. Please complete the application form and mail it in,  or you may drop the completed form off at the preschool office. Please be sure that the $100 registration form accompanies your registration paperwork. Please do not email registration forms. They will still be collected and opened in the order in which they are received.

2026-2027 Preschool Registration

Spots remaining are in Tuesday/Thursday 3’s and MWF or M-TH Prek. Extended Learning/STEM can be used to build a 5 day schedule for these classes.

Please print registration form and return to preschool office with your registration  fee. ($100 per student/$75 for siblings)

Registration forms are accepted in the order they are received with priority given to current families and Asbury UMC members.

Clubhouse is full for the 2026-2027 school year.

 Required Documents for Preschool Students and Clubhouse Students

Children medical records must be updated when there are any changes made to immunization or health status.

If no changes have been made, Preschool medical records only need to be submitted when you first register.

The following forms are for School Age Clubhouse Students Only: Clubhouse medical records are good for five years

Registration Fee

A $100.00 non-refundable registration fee per child ($75 each addl. child.) is due with the registration form.
