If you would like to register your child with Asbury Community Christian Preschool, please complete the following application and mail or drop off at the school.

For each child registered, the following forms are required:

• Tuition Express Parent Authorization
• Health Inventory
• Immunization
• Blood Lead Testing
• Discipline Policy

The following forms are for students with health concerns only:

• Medication Administration
• Asthma Action Form
• Allergy Action Plan

Registration Fee

A $100.00 non-refundable registration fee per child ($75 each addl. child.) is due with the registration form.
