Preschool Application and Required Forms

Registration forms for Asbury Community Christian Preschool are available to print below. Please complete the application form and mail it in,  or you may drop the completed form off at the preschool office. Please be sure that the $100 registration form accompanies your registration paperwork. Please do not email registration forms. They will still be collected and opened in the order in which they are received.

2025-2026 Preschool Registration  (Remaining spots in T/Th 3’s, MWF preK and M-Th PreK. Ext. learning/STEM spots available M-F

CLUBHOUSE – Full. Please call 410 757 7113 if you would like to be placed on the waitlist.

Required Documents

For each child registered, the following forms are required, and must be returned to school completed by August 15th for your child to begin school.

• Tuition Express Parent Authorization
• Health Inventory
• Medication Administration Authorization
• Blood Lead testing form
• Imunization form
• Asbury All About Me
• Asbury Community Christian Preschool and Clubhouse Behavior Agreement and Discipline Policy

The following forms are for students with health concerns only:

• Medication Administration
• Asthma Action Form
• Allergy Action plan

The following form is for School Age Students Only:

• Clubhouse All about me (All Clubhouse students must fill this out)

Registration Fee

A $100.00 non-refundable registration fee per child ($75 each addl. child.) is due with the registration form.
